You can tell yourself that it will be fine, but if your subconscious believes otherwise, your body will keep reacting.

Why Vaginismus Happens

Vaginismus is driven by the body’s stress response. When your brain perceives a threat, it activates the fight-or-flight response, which prepares your body to protect itself. With vaginismus, this response causes the pelvic floor muscles to tighten involuntarily, making penetration painful or impossible.

The threat doesn’t have to be real or logical. It could be a fear of pain, a memory of a previous difficult experience, or anxiety about sex or intimacy. Your subconscious mind doesn’t distinguish between actual danger and perceived danger. If it believes penetration is something to protect you from, it will trigger the muscles to tense up.

This is why you can’t simply decide to relax. The response is happening automatically, below your conscious awareness. And the more you try to push through or force yourself, the more your brain interprets the situation as threatening, which can make the tension worse.

For many women, there’s also an anticipation element. You expect it to hurt, so your body braces itself before anything has even happened. This creates a cycle where the fear of pain causes tension, the tension causes pain, and the pain confirms the fear.
